We understand that going to a new doctor’s office may be daunting and a bit confusing, but rest assured that we will explain what you can expect from us every step of the way. Your initial appointment will consist of a consultation explaining any diagnosis’s and applicable treatment options. Often, surgery can be performed on the same day as the consultation as needed. However, a complex medical history or treatment plan will require an evaluation and a
second appointment to provide the appropriate treatment on another day.

To help make your visit with us run as efficiently as possible, the office of Alster Dental Associates has provided our patient forms for you right here on this page! We want to ensure that you get the most out of your appointment, so feel free to fill out these forms prior to your appointment.
Current medical conditions are important to note, especially those that may necessitate premedication or special precautions during periodontal treatment. In fact, you might want to let us know about diabetes, artificial heart valves, pregnancy, hypertension, or heart murmurs when you schedule your appointment.
